12/17/2018	8:25 PM	Unruly	West South St.	Officer was met at the police department by complainant.  After speaking with all parties, Officer then returned to patrol.		Approved
12/17/2018	11:39 PM	Curfew	South Chillicothe St.	Officer was on patrol when he observed a juvenile walking on South Chillicothe Street near Beach Street. Officer then transported juvenile to his house and released him to his mother.  The juvenile was cited for Minor's Curfew.		Approved
12/17/2018	7:41 PM	Miscellaneous - Unclassified	West Main St.	Officer was dispatched to the listed location in reference to a stolen vehicle.  A short time later Officer learned the vehicle had been repossessed.  Officer then returned to patrol.		Approved

12/17/2018	6:05 PM	Unruly	West South St.	Officer was dispatched to the listed location in reference to an unruly juvenile. Officer then met with the juvenile and he agreed to go to his room and calm down before doing what he was supposed to do. Officer later returned to patrol.		Approved

Officer observed that the subject had a severe laceration just below her elbow. The medics arrived on scene and transported to subject to the hospital. 12/17/2018	8:27 AM	Non-Injury Crash	West Main St.	Officer responded to the listed location in reference to a non-injury crash. Upon arrival, Officer met with Melinda Swartz and Tamera Waldnig The crash caused minor damage to both vehicles and both were driven from the scene. Ms. Swartz was issued a citation for the unsafe lane change (PC ORD 331.14A) and Ms Waldnig was found to have a suspended license and was issued a citation for No Operator's License (ORC 4510.12). 12/18/2018	2:30 PM	Other Sex Offense	South Jefferson Ave.	Officer was advised of a sexual assault that occurred at the listed location on an unknown date. This incident was referred to MCDOJFS.	McDonald's	Approved
12/18/2018	5:45 PM	Theft - Misdemeanor	South Jefferson Ave.	Officer received a blue Verizon LG phone from MCSO Deputy who advised that he confiscated the phone from subject at the Jonathan Alder High School. Subject admitted that he stole the phone from the Dollar General.  The manager at Dollar General did not wish to pursue with criminal charges, but advised that he wanted subject trespassed from the business. Subject was advised not to return to the business or he would be charged for Criminal Trespass.	Dollar General Store	Approved
12/18/2018	7:21 PM	Warrant - Felony	West Bigelow Ave.	While on routine patrol, Officer observed Kenneth Mount's vehicle parked on the lot of the listed location. Having prior knowledge that Kenneth had an entered warrant out of the Madison County Common Pleas for Aggravated Possession of Drugs (confirmed), Kenneth was placed into custody without incident and transported to the Tri-County Regional Jail. 12/21/2018	3:26 AM	Suspicious Person	North Chillicothe St.	Officer was on patrol when Officer observed subject walking northbound near 370 North Chillicothe Street. Due to the rain and subject not being very visible, Officer transported subject to the residence, Officer then returned to patrol.		Approved
12/21/2018	4:28 PM	Assist Other Agency	US Rt. 33	Officer was requested by Ohio State Highway Patrol (Post 80) to perform a narcotic sniff of a vehicle involved in a traffic stop on US 33 eastbound near the Rest Area. After assistance was no longer needed, Officer returned to patrol.		Approved
12/21/2018	7:14 PM	Attempted Suicide	Maple Ct.	Officer responded to the listed location in reference to a suicide attempt.  Subject was taken to hospital.  A CIT report will be completed for this incident and a copy of this report will be forwarded to the Madison County Department of Jobs and Family Services.
